As a Caring for Colorado grantee you will be asked for periodic grant reports. Each awarded grant has specific requirements for reporting on progress, challenges and accomplishments.

Caring for Colorado Foundation requires grantees to request approval prior to making changes to an approved grant project plan, timeline, budget or contract.
After first talking with your Caring for Colorado staff contact, please download and complete the Grant Amendment Request Form and submit to your CFC contact via email.
Once submitted, we will review your request and be in touch with you with any questions or issues we would like to discuss further.
